In this post we shall discuss about Keloid formation and treatment<\/p>\n\n\n\n

Keloid is a defect in maturation and stabilization of collagen fibrils as normal collagen bundles are absent<\/p>\n\n\n\n

It is common in females<\/p>\n\n\n\n

Mostly seen in black people<\/p>\n\n\n\n

Genetically predisposed and very rare in Caucasians<\/p>\n\n\n\n

Keloid grows even after 6 months. It extends into adjacent normal skin. It is brownish black\/pinkish black in color, painful, itchy and hyperaesthetic.<\/p>\n\n\n\n

Keloids can be associated ot Ehlers Danlos Syndrome or Scleroderma<\/p>\n\n\n\n

Spontaneous keloid is formed when an unnoticed trauma heals without scar formation.<\/p>\n\n\n\n

Some keloids don’t grow after an initial growth<\/p>\n\n\n\n

Keloid contains proliferating immature blood vessels and type III thick collagen stroma and immature fibroblasts.<\/p>\n\n\n\n

Keloids are common over sternum along with upper arm, chest wall, front of lower neck<\/p>\n\n\n\n

Precipitating Factors<\/strong><\/p>\n\n\n\n

Black People<\/p>\n\n\n\n

Tuberculosis patient<\/p>\n\n\n\n

Vaccination site<\/p>\n\n\n\n

\n\n\n\n

Complications<\/strong><\/p>\n\n\n\n

Infection<\/p>\n\n\n\n

Ulceration<\/p>\n\n\n\n

\n\n\n\n

Treatment<\/strong><\/p>\n\n\n\n

Excision and skin grafting can be done<\/p>\n\n\n\n

Vit. E\/ palm oil massage<\/p>\n\n\n\n

Intrakeloidal triamcinolone injection once in 7-10 days followed by excision. The usual sequence is Steroid injection – Excision – Steroid Injection<\/p>\n\n\n\n

Methotrexate and Vit. A<\/p>\n\n\n\n

Compression dressings with silicone gel sheets<\/p>\n\n\n\n

Laser therapy<\/p>\n\n\n\n

Intra lesional excision retaining the scar margin could prevent recurrence. It is preferred over only excision.<\/p>\n\n\n\n
